What is a Kidney Cyst?
A kidney cyst is a round, fluid-filled sac that forms in or on the kidney. They are extremely common — approximately 50% of people over 50 have at least one kidney cyst. Most cysts are harmless and require no treatment.
Types (Bosniak Classification)
- Bosniak I: Simple cyst, no cancer risk, no treatment needed
- Bosniak II: Minimal complexity, cancer risk 0-5%
- Bosniak IIF: Requires follow-up imaging, cancer risk 5-15%
- Bosniak III: Surgery recommended, cancer risk 40-60%
- Bosniak IV: Surgery required, cancer risk 80-100%
Symptoms
- Usually asymptomatic
- Flank pain (large cysts)
- Blood in urine
- High blood pressure
- Fever (infected cyst)
Treatment
- Simple cysts: Annual ultrasound follow-up
- Complex cysts (Bosniak III-IV): Robotic/laparoscopic partial nephrectomy
- Symptomatic cysts: Aspiration + sclerotherapy or laparoscopic decortication
